Medication Refills

To refill your pet's medication, please either:

- Call us at (253) 839-4744 during normal business hours or,
- Send us an email message at Refill@CrestwoodAnHosp.com with the word REFILL in the subject line

To serve you better and refill your medication faster, we'll need to know the following information:
 

		Pet owner's name
		Pet's name
		Medication name and quantity needed
		Telephone number where the pet owner can be reached (in case we have questions)
We'll call you when your refill is ready for pick up.
