Welcome to Crestwood Animal Hospital!

We are accepting new patients, at this time! Patients at Crestwood are treated as family. If you ever have questions, or are just not sure about an issue with your pet, please feel free to call us!

Medical records

After making your appointment with our receptionist, we ask that you collect any medical records you may have from your previous veterinary visits. (If you have a new animal without records, then don’t worry.) If your pet has been seen more than once at any other veterinary clinic, we ask that you call the clinic and have them fax us all previous records. Our fax number is (253) 941-1650. The doctor will review the records before your appointment in order to become familiar with your pet’s history and any preexisting conditions.

Appointments

We ask that you arrive at least 15-minutes early to fill out our new patient paperwork. Arriving early allows us to get the information we need and give us time to get acquainted, and yet leave enough time for a complete examination and office call.

To make an appointment, please call our friendly receptionist at (253) 839-4744. We make every effort to accommodate your convenience for appointments. Our extended hours will make it more convenient for you with evening and Saturday hours.

Office location

For your convenience, there is a map on the About Us page that will help you locate us. When coming to our hospital from the south, once you get on Pacific Highway S., you will find us on the right, just before the corner of Pacific Highway S. and 288^th, and across the street from Verrazano’s Restaurant. If coming from the north, turn left at the stoplight at 288^th and Pacific Highway S. Then turn right into our alley which is just past the fence at the end of the Shell station. Proceed down the alley, to our parking lot, which is located on the south side of our building.

Pet safety

We ask that all dogs be kept on leashes and that all cats and small mammals be kept in a carrier until they enter the exam rooms. This is for your pet’s safety and ensure that they don't run away or getting into traffic.
